        "resumen" => "<span class="elsevierStyleSectionTitle">Objetivo</span><p id="spar0005" class="elsevierStyleSimplePara elsevierViewall">La torsi&#243;n primaria del epipl&#243;n mayor es una causa infrecuente de abdomen agudo en los ni&#241;os&#46; Se realiza un estudio retrospectivo con el fin de establecer su incidencia y caracter&#237;sticas cl&#237;nicas&#46;</p> <span class="elsevierStyleSectionTitle">Pacientes y m&#233;todos</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Se revisan las historias cl&#237;nicas de los ni&#241;os intervenidos de torsi&#243;n de epipl&#243;n en el Hospital Infantil Universitario Miguel Servet de Zaragoza en los &#250;ltimos 25 a&#241;os&#46; Se analizaron los siguientes datos&#58; edad de presentaci&#243;n&#44; sexo&#44; factores predisponentes&#44; sintomatolog&#237;a&#44; exploraciones complementarias efectuadas&#44; tratamiento y evoluci&#243;n&#46;</p> <span class="elsevierStyleSectionTitle">Resultados</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">En los 15 pacientes con torsi&#243;n de epipl&#243;n la raz&#243;n var&#243;n&#58;mujer fue de 2&#58;1&#46; La sintomatolog&#237;a fue similar a la de la apendicitis aguda con ciertas peculiaridades como un m&#225;s largo per&#237;odo de evoluci&#243;n en el momento del diagn&#243;stico&#44;una fiebre y leucocitosis menores de lo que ser&#237;a de esperar en una apendicitis con esas horas de evoluci&#243;n y&#44; en 12 casos&#44; la ausencia de v&#243;mitos&#46; Tras el tratamiento quir&#250;rgico la evoluci&#243;n fue satisfactoria&#46; En 13 casos la torsi&#243;n fue primaria y en 2 casos secundaria a hernia inguinal y linfangioma qu&#237;stico de epipl&#243;n&#44; respectivamente&#46; Se discuten la etiolog&#237;a&#44; la patogenia y los problemas diagn&#243;sticos y terap&#233;uticos de este proceso&#46;</p> <span class="elsevierStyleSectionTitle">Conclusiones</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">En todos los casos de torsi&#243;n primaria el diagn&#243;stico cl&#237;nico fue de apendicitis aguda&#46; A pesar de estar cl&#225;sicamente asociada a la obesidad s&#243;lo en 1 de los 13 casos de torsi&#243;n primaria se encontr&#243; un peso mucho mayor de la media para su edad y sexo en nuestra regi&#243;n&#44; y en ning&#250;n caso hubo una malformaci&#243;n anat&#243;mica claramente asociada&#46;</p>"

        "resumen" => "<span class="elsevierStyleSectionTitle">Objective</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">Primary torsion of the greater omentum is an infrequent cause of acute abdomen in children&#46; A retrospective review was conducted to establish the prevalence and clinical features of omental torsion as a cause of acute abdominal pain in childhood&#46;</p> <span class="elsevierStyleSectionTitle">Patients and methods</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">We reviewed the clinical histories of the children given surgical treatment for torsion of the greater omentum in our hospital in the last 25 years&#46; The following data were studied&#58; age at presentation&#44; sex&#44; predisposing factors&#44; symptomatology&#44; complementary investigations&#44; treatment and evolution&#46;</p> <span class="elsevierStyleSectionTitle">Results</span><p id="spar00235" class="elsevierStyleSimplePara elsevierViewall">The male&#58;female ratio among the 15 patients who underwent surgery was 2&#58;1&#46; Symptomology was similar to that of acute appendicitis with certain peculiarities such as a longer period of evolution at the moment of diagnosis&#44; lower temperature and leucocytosis lower than would be expected in appendicitis at the same time of evolution and&#44; in 12 patients&#44; absence of vomiting&#46; After surgical treatment evolution was satisfactory&#46; Torsion was primary in 13 patients&#44; secondary to inguinal hernia in 1 and secondary to cystic lymphangioma of the omentum in 1&#46; The etiology and pathogenesis&#44; as well as the diagnostic and therapeutic problems of this process&#44; are discussed&#46;</p> <span class="elsevierStyleSectionTitle">Conclusions</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">In all the patients with primary torsion the clinical diagnosis was of acute appendicitis&#46; Although primary torsion is classically associated with obesity&#44; only 1 of the 13 patients weighed significantly more than the average for the same age and sex in our region and none of the patients showed a clearly associated anatomic malformation&#46;</p>"

Objetivo

La torsión primaria del epiplón mayor es una causa infrecuente de abdomen agudo en los niños. Se realiza un estudio retrospectivo con el fin de establecer su incidencia y características clínicas.

Se revisan las historias clínicas de los niños intervenidos de torsión de epiplón en el Hospital Infantil Universitario Miguel Servet de Zaragoza en los últimos 25 años. Se analizaron los siguientes datos: edad de presentación, sexo, factores predisponentes, sintomatología, exploraciones complementarias efectuadas, tratamiento y evolución.

Resultados

En los 15 pacientes con torsión de epiplón la razón varón:mujer fue de 2:1. La sintomatología fue similar a la de la apendicitis aguda con ciertas peculiaridades como un más largo período de evolución en el momento del diagnóstico,una fiebre y leucocitosis menores de lo que sería de esperar en una apendicitis con esas horas de evolución y, en 12 casos, la ausencia de vómitos. Tras el tratamiento quirúrgico la evolución fue satisfactoria. En 13 casos la torsión fue primaria y en 2 casos secundaria a hernia inguinal y linfangioma quístico de epiplón, respectivamente. Se discuten la etiología, la patogenia y los problemas diagnósticos y terapéuticos de este proceso.

Conclusiones

En todos los casos de torsión primaria el diagnóstico clínico fue de apendicitis aguda. A pesar de estar clásicamente asociada a la obesidad sólo en 1 de los 13 casos de torsión primaria se encontró un peso mucho mayor de la media para su edad y sexo en nuestra región, y en ningún caso hubo una malformación anatómica claramente asociada.

Objective

Primary torsion of the greater omentum is an infrequent cause of acute abdomen in children. A retrospective review was conducted to establish the prevalence and clinical features of omental torsion as a cause of acute abdominal pain in childhood.

Patients and methods

We reviewed the clinical histories of the children given surgical treatment for torsion of the greater omentum in our hospital in the last 25 years. The following data were studied: age at presentation, sex, predisposing factors, symptomatology, complementary investigations, treatment and evolution.

Results

The male:female ratio among the 15 patients who underwent surgery was 2:1. Symptomology was similar to that of acute appendicitis with certain peculiarities such as a longer period of evolution at the moment of diagnosis, lower temperature and leucocytosis lower than would be expected in appendicitis at the same time of evolution and, in 12 patients, absence of vomiting. After surgical treatment evolution was satisfactory. Torsion was primary in 13 patients, secondary to inguinal hernia in 1 and secondary to cystic lymphangioma of the omentum in 1. The etiology and pathogenesis, as well as the diagnostic and therapeutic problems of this process, are discussed.

Conclusions

In all the patients with primary torsion the clinical diagnosis was of acute appendicitis. Although primary torsion is classically associated with obesity, only 1 of the 13 patients weighed significantly more than the average for the same age and sex in our region and none of the patients showed a clearly associated anatomic malformation.
