Nódulo cutáneo como forma de presentación de una leucemia/linfoma linfoblástico de estirpe B

La afectación cutánea primaria por una leucemia/linfoma linfoblástico de células B es una rareza en la infancia. Se presenta el caso de una niña de 11 años y medio a la que 5 meses antes de ser remitida a nuestro centro se le había extirpado un pequeño bultoma subcutáneo en la región glútea derecha, diagnosticado anatomopatológicamente de proliferación linfoide indicativa de linfoma de células grandes. A la exploración llamaba la atención la existencia de pequeños nódulos próximos a la cicatriz de la tumoración previamente extirpada. En el hemograma presentaba 2,7 × 109/l leucocitos con 0,5 × 109/l neutrófilos y el resto de las series normales; así como exploraciones complementarias sin hallazgos patológicos de interés. Se realizó entonces aspirado de médula ósea (AMO) en el que destacaba la presencia de un 52 % de blastos con características inmunofenotípicas y morfológicas de leucemia linfoblástica aguda (LLA) B común, subtipo L2 de la clasificación FAB (French-American-British Cooperative Working Group). Tras 10 meses de finalizada la poliquimioterapia la paciente se encuentra en remisión completa. Queremos destacar el hecho de que un nódulo cutáneo de evolución tórpida puede ser la forma de presentación de un linfoma linfoblástico o de una LLA.

Primary cutaneous involvement in B-cell lymphoblastic leukemia/lymphoma is rare in childhood. We present the case of an eleven and a half year old girl who, five months prior to being referred to our center, had undergone surgery to remove a small gluteal tumor diagnosed histopathologically as lymphoid proliferation suggestive of large cell lymphoma. On examination the presence of small nodes close to the scar where the tumor had previously been removed was observed. Hemogram revealed 2.7 × 109/l white blood cells with 0.5 × 109/l neutrophils; the remaining series and complementary investigations were normal. Bone marrow aspiration revealed 52 % blastic cells with immunophenotype and morphological characteristics of common (B-cell) acute lymphoblastic leukemia with L2 subtype in the French-American-British (FAB) classification. Ten months after finishing polychemotherapy, the patient is now in complete remission. We would like to highlight that a small slow-growing cutaneous node could be the presenting form of lymphoblastic lymphoma or acute lymphoblastic leukemia.
