Enfermedad de Graves en pacientes con síndrome de Down

El propósito de esta nota clínica es la presentación de 3 pacientes con la infrecuente asociación de síndrome de Down y enfermedad de Graves. Dos de ellos permanecían asintomáticos en el momento del diagnóstico, mientras que el tercero presentaba bocio, nerviosismo, pérdida de peso y taquicardia. Además de la sintomatología característica de hipertiroidismo, en este paciente se objetivó insuficiencia cardíaca derecha e hipertransaminasemia, que desaparecieron en el transcurso del tratamiento antitiroideo.

Debido a la rareza de la enfermedad de Graves en la infancia, así como la forma de presentación de uno de nuestros pacientes, se presentan 3 casos clínicos de enfermedad de Graves en niños con síndrome de Down, destacando la importancia de valorar la función tiroidea periódicamente en los niños con esta cromosomopatía, no sólo para la detección de hipotiroidismo.

Three patients showing the rare association of Down syndrome and Graves‚ disease are reported. While two of the patients were asymptomatic, the third showed goiter, nervousness, weight loss, and tachycardia. In addition to the typical features of hyperthyroidism, this patient showed right heart failure and hypertransaminasemia, which disappeared with antithyroid treatment.

Because Graves‚ disease is rare in children, and the clinical presentation was unusual in one of our patients, we report three patients with Graves‚ disease and Down syndrome, and emphasize the importance of periodic evaluation of thyroid function in children with Down syndrome not only to detect hypothyroidism.
