    "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">El abordaje de la enfermedad causada por el SARS-COV-2 &#40;COVID-19&#41; ha ido variando desde el inicio de la pandemia producida por dicho virus&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">El desconocimiento inicial sobre los mecanismos de actuaci&#243;n del virus&#44; la ausencia de bibliograf&#237;a disponible y la gravedad del cuadro en pacientes adultos derivaron en pautas de tratamiento durante la primera ola que han ido cambiando posteriormente a medida que aumentaba el conocimiento de la enfermedad<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a>&#46; En este estudio descriptivo retrospectivo realizado en Urgencias Pedi&#225;tricas del Hospital Universitario La Paz de Madrid analizamos y comparamos las caracter&#237;sticas de los pacientes y su manejo entre la primera ola &#40;31&#47;03&#47;2020-26&#47;06&#47;2020&#41; y la segunda ola &#40;20&#47;07&#47;2020-30&#47;11&#47;2020&#41;&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">En la primera ola fueron identificados 74 pacientes positivos para SARS-CoV-2<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a> y en la segunda se diagnosticaron 180 pacientes&#46; Los principales datos se exponen en la <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">La mediana de edad de la totalidad de pacientes fue de 62&#44;7 meses &#40;RI&#58; 254&#41;&#44; el 56&#44;7&#37; fueron varones&#46; No hubo diferencias significativas en la edad y sexo entre las olas&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Inicialmente&#44; todos los pacientes fueron identificados como positivos mediante t&#233;cnica PCR&#44; dado que era la &#250;nica disponible<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">Durante la segunda ola&#44; con la aparici&#243;n de los test de ant&#237;geno se identificaron 40 positivos &#40;22&#44;2&#37;&#41;<span class="elsevierStyleItalic">&#46;</span></p><p id="par0035" class="elsevierStylePara elsevierViewall">En la primera ola &#250;nicamente se realiz&#243; PCR de SARS-CoV-2 a pacientes que requer&#237;an ingreso o que ten&#237;an s&#237;ntomas y patolog&#237;a de base<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; En la segunda ola se realizaron test diagn&#243;sticos a todos los pacientes con cl&#237;nica compatible&#44; a los que ingresaron y a ni&#241;os asintom&#225;ticos con alg&#250;n conviviente positivo&#46; Esto se refleja en que el porcentaje de pacientes con patolog&#237;a de base fue mayor durante la primera ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; Adem&#225;s&#44; el porcentaje de asintom&#225;ticos diagnosticados en la primera ola fue significativamente menor que en la segunda&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">El nivel de triaje fue mayor en la primera ola &#40;siendo el m&#225;s frecuente el nivel<span class="elsevierStyleHsp" style=""></span>3&#41;&#44; versus nivel<span class="elsevierStyleHsp" style=""></span>4 de triaje durante la segunda ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">En cuanto a los s&#237;ntomas&#44; se objetivaron diferencias estad&#237;sticamente significativas entre las dos olas en los s&#237;ntomas de tos y rechazo de las tomas&#44; m&#225;s frecuentes ambos en la primera ola<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a>&#46; En la segunda ola destaca el aumento significativo de diagn&#243;sticos en pacientes sin s&#237;ntomas sugestivos de COVID-19&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Los diagn&#243;sticos m&#225;s frecuentes en ambas olas fueron infecci&#243;n aguda de v&#237;as respiratorias superiores y s&#237;ndrome febril sin foco<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; En la primera ola se diagnosticaron 8 neumon&#237;as y ninguna en la segunda ola&#46; En la primera ola se realizaba radiograf&#237;a a la mayor&#237;a de los pacientes extrapolando el algoritmo de manejo de los pacientes adultos&#46; En la segunda ola&#44; tras ir adquiriendo cierta experiencia y viendo que las neumon&#237;as eran muy infrecuentes en ni&#241;os se dej&#243; de hacer dicha prueba&#46; Probablemente en la primera ola se sobreestimara el porcentaje de neumon&#237;as considerando infiltrados leves como posibles focos neum&#243;nicos&#46; Se revis&#243; si los pacientes reconsultaron en urgencias en las 72 horas posteriores a la consulta inicial para descartar la presencia de complicaciones secundarias a la infecci&#243;n&#44; entre ellas neumon&#237;as&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">El n&#250;mero de ingresos y la duraci&#243;n de los mismos fue significativamente menor en el segundo per&#237;odo &#40;primera ola mediana de 4<span class="elsevierStyleHsp" style=""></span>d&#237;as con RI&#58; 29<span class="elsevierStyleHsp" style=""></span>d&#237;as&#59; segunda ola mediana 2<span class="elsevierStyleHsp" style=""></span>d&#237;as con RI&#58; 7&#41;<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">No hubo diferencias en la necesidad de ingreso en Cuidados Intensivos&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall">Hemos analizado por separado el abordaje de los pacientes sintom&#225;ticos con infecci&#243;n confirmada en ambas olas&#46; Durante la primera ola el manejo fue m&#225;s invasivo&#44; realiz&#225;ndose anal&#237;tica sangu&#237;nea y radiograf&#237;a de t&#243;rax en un n&#250;mero significativamente mayor&#46; En ning&#250;n caso se realiz&#243; TAC tor&#225;cica &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">En cuanto al tratamiento&#44; se observan marcadas diferencias entre las dos olas<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; El empleo de antibioterapia fue mayor durante la primera ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; En relaci&#243;n con otros tratamientos utilizados durante la primera ola &#40;hidroxicloroquina&#44; antivirales&#44; tocilizumab&#41;&#44; ninguno se utiliz&#243; en la segunda&#44; destacando la hidroxicloroquina&#44; que se utiliz&#243; en el 44&#44;6&#37; de los pacientes en la primera ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; Tambi&#233;n fue menor el uso de oxigenoterapia &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>&#41;&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">El desconocimiento y el miedo a la posible gravedad de la infecci&#243;n en ni&#241;os conllev&#243; mayor n&#250;mero de reconsultas durante la primera ola&#44; siendo esta diferencia estad&#237;sticamente significativa&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">Alrededor del 50&#37; de los pacientes identificaron alg&#250;n contacto estrecho&#44; siendo el principal foco de contagio el entorno familiar&#46; Destaca que&#44; a pesar de que en la segunda ola se encontraban los centros escolares en funcionamiento&#44; apenas se identificaron casos de contagios en los mismos&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">Como conclusiones de nuestro estudio nos gustar&#237;a destacar que el colapso sanitario sufrido durante la primera ola en Madrid produjo un infradiagn&#243;stico de la infecci&#243;n por SARS CoV-2 en ni&#241;os&#44; dado que &#250;nicamente se realizaban pruebas diagn&#243;sticas en pacientes con patolog&#237;a de base o que precisaban ingreso&#46; Esto se refleja en las caracter&#237;sticas de los pacientes diagnosticados en ambas olas&#44;     "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">El abordaje de la enfermedad causada por el SARS-COV-2 &#40;COVID-19&#41; ha ido variando desde el inicio de la pandemia producida por dicho virus&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">El desconocimiento inicial sobre los mecanismos de actuaci&#243;n del virus&#44; la ausencia de bibliograf&#237;a disponible y la gravedad del cuadro en pacientes adultos derivaron en pautas de tratamiento durante la primera ola que han ido cambiando posteriormente a medida que aumentaba el conocimiento de la enfermedad<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a>&#46; En este estudio descriptivo retrospectivo realizado en Urgencias Pedi&#225;tricas del Hospital Universitario La Paz de Madrid analizamos y comparamos las caracter&#237;sticas de los pacientes y su manejo entre la primera ola &#40;31&#47;03&#47;2020-26&#47;06&#47;2020&#41; y la segunda ola &#40;20&#47;07&#47;2020-30&#47;11&#47;2020&#41;&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">En la primera ola fueron identificados 74 pacientes positivos para SARS-CoV-2<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a> y en la segunda se diagnosticaron 180 pacientes&#46; Los principales datos se exponen en la <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">La mediana de edad de la totalidad de pacientes fue de 62&#44;7 meses &#40;RI&#58; 254&#41;&#44; el 56&#44;7&#37; fueron varones&#46; No hubo diferencias significativas en la edad y sexo entre las olas&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Inicialmente&#44; todos los pacientes fueron identificados como positivos mediante t&#233;cnica PCR&#44; dado que era la &#250;nica disponible<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">Durante la segunda ola&#44; con la aparici&#243;n de los test de ant&#237;geno se identificaron 40 positivos &#40;22&#44;2&#37;&#41;<span class="elsevierStyleItalic">&#46;</span></p><p id="par0035" class="elsevierStylePara elsevierViewall">En la primera ola &#250;nicamente se realiz&#243; PCR de SARS-CoV-2 a pacientes que requer&#237;an ingreso o que ten&#237;an s&#237;ntomas y patolog&#237;a de base<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; En la segunda ola se realizaron test diagn&#243;sticos a todos los pacientes con cl&#237;nica compatible&#44; a los que ingresaron y a ni&#241;os asintom&#225;ticos con alg&#250;n conviviente positivo&#46; Esto se refleja en que el porcentaje de pacientes con patolog&#237;a de base fue mayor durante la primera ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; Adem&#225;s&#44; el porcentaje de asintom&#225;ticos diagnosticados en la primera ola fue significativamente menor que en la segunda&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">El nivel de triaje fue mayor en la primera ola &#40;siendo el m&#225;s frecuente el nivel<span class="elsevierStyleHsp" style=""></span>3&#41;&#44; versus nivel<span class="elsevierStyleHsp" style=""></span>4 de triaje durante la segunda ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">En cuanto a los s&#237;ntomas&#44; se objetivaron diferencias estad&#237;sticamente significativas entre las dos olas en los s&#237;ntomas de tos y rechazo de las tomas&#44; m&#225;s frecuentes ambos en la primera ola<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a>&#46; En la segunda ola destaca el aumento significativo de diagn&#243;sticos en pacientes sin s&#237;ntomas sugestivos de COVID-19&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Los diagn&#243;sticos m&#225;s frecuentes en ambas olas fueron infecci&#243;n aguda de v&#237;as respiratorias superiores y s&#237;ndrome febril sin foco<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; En la primera ola se diagnosticaron 8 neumon&#237;as y ninguna en la segunda ola&#46; En la primera ola se realizaba radiograf&#237;a a la mayor&#237;a de los pacientes extrapolando el algoritmo de manejo de los pacientes adultos&#46; En la segunda ola&#44; tras ir adquiriendo cierta experiencia y viendo que las neumon&#237;as eran muy infrecuentes en ni&#241;os se dej&#243; de hacer dicha prueba&#46; Probablemente en la primera ola se sobreestimara el porcentaje de neumon&#237;as considerando infiltrados leves como posibles focos neum&#243;nicos&#46; Se revis&#243; si los pacientes reconsultaron en urgencias en las 72 horas posteriores a la consulta inicial para descartar la presencia de complicaciones secundarias a la infecci&#243;n&#44; entre ellas neumon&#237;as&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">El n&#250;mero de ingresos y la duraci&#243;n de los mismos fue significativamente menor en el segundo per&#237;odo &#40;primera ola mediana de 4<span class="elsevierStyleHsp" style=""></span>d&#237;as con RI&#58; Durante la primera ola el manejo fue m&#225;s invasivo&#44; realiz&#225;ndose anal&#237;tica sangu&#237;nea y radiograf&#237;a de t&#243;rax en un n&#250;mero significativamente mayor&#46; En ning&#250;n caso se realiz&#243; TAC tor&#225;cica &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">En cuanto al tratamiento&#44; se observan marcadas diferencias entre las dos olas<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; El empleo de antibioterapia fue mayor durante la primera ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; En relaci&#243;n con otros tratamientos utilizados durante la primera ola &#40;hidroxicloroquina&#44; antivirales&#44; tocilizumab&#41;&#44; ninguno se utiliz&#243; en la segunda&#44; destacando la hidroxicloroquina&#44; que se utiliz&#243; en el 44&#44;6&#37; de los pacientes en la primera ola<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">2&#44;3</span></a>&#46; Tambi&#233;n fue menor el uso de oxigenoterapia &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>&#41;&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">El desconocimiento y el miedo a la posible gravedad de la infecci&#243;n en ni&#241;os conllev&#243; mayor n&#250;mero de reconsultas durante la primera ola&#44; siendo esta diferencia estad&#237;sticamente significativa&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">Alrededor del 50&#37; de los pacientes identificaron alg&#250;n contacto estrecho&#44; siendo el principal foco de contagio el entorno familiar&#46; Destaca que&#44; a pesar de que en la segunda ola se encontraban los centros escolares en funcionamiento&#44; apenas se identificaron casos de contagios en los mismos&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">Como conclusiones de nuestro estudio nos gustar&#237;a destacar que el colapso sanitario sufrido durante la primera ola en Madrid produjo un infradiagn&#243;stico de la infecci&#243;n por SARS CoV-2 en ni&#241;os&#44; dado que &#250;nicamente se realizaban pruebas diagn&#243;sticas en pacientes con patolog&#237;a de base o que precisaban ingreso&#46; Esto se refleja en las caracter&#237;sticas de los pacientes diagnosticados en ambas olas&#44; observando en la primera ola mayor n&#250;mero de pacientes con patolog&#237;a de base&#44; un nivel de triaje de mayor prioridad y mayor n&#250;mero de ingresos&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">Adem&#225;s&#44; el desconocimiento inicial de esta patolog&#237;a influy&#243; en un abordaje m&#225;s invasivo a nivel de pruebas diagn&#243;sticas&#44; ingresos&#44; tratamientos y reconsultas durante la primera ola&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">A pesar de que durante la segunda ola los centros educativos permanecieron abiertos&#44; se observa que la mayor&#237;a de contactos fueron intrafamiliares&#46;</p></span>"
