Sin embargo, en relación con el transporte interhospitalario aún es muy escasa la evidencia que apoye su uso<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">3</span></a>. Presentamos los resultados de un estudio descriptivo sobre el uso de la EC en el contexto del traslado interhospitalario de pacientes críticos pediátricos y neonatales, remarcando su utilidad.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Se registraron las EC realizadas por una unidad terrestre de soporte vital avanzado pediátrico y neonatal del Sistema de Emergencias Médicas de Cataluña entre el 1 de noviembre de 2019 y el 29 de febrero de 2020. Dichos registros se realizaron prospectivamente por parte del pediatra,que efectuó la exploración en una base de datos creada al efecto. Todos cuentan con un curso de formación teórico-práctico de EC y experiencia profesional en la estabilización y traslado exclusivamente de pacientes críticos pediátricos y neonatales. Las variables incluidas son: demográficas, diagnóstico inicial, principal compromiso detectado, lugar de realización, sondas y preconfigurados utilizados, tiempo invertido, áreas estudiadas, protocolo utilizado, hallazgos encontrados, diagnóstico final e implicaciones en el manejo del paciente. Las imágenes se obtuvieron con el ecógrafo SonoSite Edge II® y mediante sondas lineales (13-6<span class="elsevierStyleHsp" style=""></span>MHz) y sectoriales (8-4 y 5-1<span class="elsevierStyleHsp" style=""></span>MHz).</p><p id="par0015" class="elsevierStylePara elsevierViewall">Durante el periodo de estudio se trasladaron un total de 270 pacientes. Se realizaron 26 EC en 23 pacientes (8,5%). Las principales características clínicas y demográficas se presentan en la <a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a> y los principales resultados de las ecografías en la <a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a>. Se realizaron 14 ecografías (53,8%) en la ambulancia y 12 (46,2%) en el hospital emisor, sin hallar diferencias significativas en el tipo de pacientes. La mediana del tiempo invertido en su realización fue de 5<span class="elsevierStyleHsp" style=""></span>minutos (rango 3-10). El 53,8% (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>14) de las EC conllevan un cambio en la actitud terapéutica o en el diagnóstico del paciente: 3 drenajes de neumotórax, 3 aumentos del soporte hemodinámico (volumen y/o fármacos simpaticomiméticos), 3 optimizaciones del soporte respiratorio al descartar neumotórax, se evitó expandir volemia en 2 pacientes por ausencia de signos de hipovolemia y la realización de una radiografía de tórax y se modificó el diagnóstico a 2 pacientes.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">La EC ha demostrado previamente cómo la toma de decisiones terapéuticas al sumar sus hallazgos puede mejorar los resultados clínicos<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">4</span></a>. Se observa además su capacidad para indicar técnicas (como el drenaje de neumotórax) o modificar diagnósticos.</p><p id="par0025" class="elsevierStylePara elsevierViewall">La escasa literatura sobre el uso de la EC en transporte se centra en pacientes neonatales<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">5</span></a>, siendo solo un tercio de los pacientes explorados en nuestro estudio, poniendo así de manifiesto la importancia también en el paciente pediátrico.</p><p id="par0030" class="elsevierStylePara elsevierViewall">La EC en transporte no está exenta de limitaciones. La más importante es la formación necesaria para evitar errores en la interpretación de resultados. No obstante, la capacitación de personal no entrenado es posible en un tiempo razonable si se sigue una metodología adecuada<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">6</span></a>. Otra limitación es la necesidad de sopesar la relevancia de la información que se puede obtener con la demora del traslado que supone su uso. Al tratarse de un contexto muy diferente al de una unidad de críticos (menos personal y recursos, espacios más reducidos y gran importancia de los tiempos invertidos), la EC debe reservarse para aquellos casos donde, ante sospechas concretas, el resultado comporte un cambio de actitud, como puede ocurrir en ciertas situaciones clínicas como el shock, donde de manera significativa solo un paciente no fue sometido a estudio.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Como principales limitaciones del estudio destacamos el pequeño tamaño muestral que obliga a interpretar los resultados con cautela. El bajo porcentaje de pacientes a los que se realizó una EC pudo deberse a que el periodo de estudio comprendió la epidemia de bronquiolitis. Estos no suelen ser tributarios de su uso en transporte, salvo por necesidad de un diagnóstico diferencial o descartar complicaciones (neumotórax). Otra limitación ha sido la imposibilidad para corroborar si las modificaciones terapéuticas o diagnósticas realizadas han supuesto un beneficio real en términos de estancia hospitalaria, mortalidad o disminución del <span class="elsevierStyleItalic">score</span> de gravedad al ingreso.</p><p id="par0040" class="elsevierStylePara elsevierViewall">En conclusión, estos resultados muestran que un porcentaje significativo de pacientes seleccionados se pueden beneficiar del uso de la ecografía en el transporte interhospitalario sin prolongar los tiempos de estabilización. Se debe individualizar su indicación con el fin de evitar demoras en el traslado. Los pacientes con compromiso respiratorio o hemodinámico fueron los más beneficiados.
