Tráquea cartilaginosa completa congénita en una niña con síndrome de Crouzon

Una niña de 4 meses con síndrome de Crouzon requirió una traqueostomía por enfermedad obstructiva respiratoria alta, ocasionada por las dismorfias craneofaciales propias de su malformación. En la intervención se observó la sustitución de los anillos traqueales por un cilindro cartilaginoso uniforme. Estos hallazgos se comprobaron en la broncoscopia postoperatoria. La corrección quirúrgica craneofacial posterior permitió la descanulación de la paciente a los 10 meses de vida, y en la actualidad se encuentra asintomática. La tráquea cartilaginosa completa es una malformación congénita muy poco frecuente que se ha descrito de forma exclusiva en pacientes con síndromes craneosinostósicos. Creemos que debe descartarse enfermería traqueobronquial en aquellos pacientes con sintomatología respiratoria no atribuible a trastornos obstructivos de la vía respiratoria alta.

A tracheostomy was performed in a 4-month-old girl with Crouzon's syndrome because of upper respiratory obstruction. During the procedure the absence of tracheal rings was observed. These findings were confirmed by postoperative bronchoscopy. Subsequent surgical correction of the patient's craniofacial anomalies enabled decannulation when the patient was 10 months old. Complete cartilaginous trachea is very rare and is always associated with craniosynostotic syndromes. Tracheobronchial anomalies should be investigated in patients whose respiratory symptoms are not due to upper airway obstruction.
